As of October 2016, patient health summary visit data is now available in CCHC to both patients and providers from all seven County Health Department clinics.
Gastroenterology Reports Now Available
As of May 2017, gastroenterology reports from the Community Hospital of Monterey are now available to patients and providers.
Tyler Heart Institute – Cardiology Reports Now Available
As of 11/10/2016, cardiology reports from the Tyler Heart Institute are now available to patients and providers. Four days after the report is finalized by your provider, the report will be available to you in your CCHC patient portal record.
You can now access your child’s medical records on CCHC
As of November 1st, you can now access your child’s medical record on CCHC. Parents have the ability to view their child’s health information, within the CCHC patient portal, for children up to age 10. Due to various state and federal laws regarding sensitive and private health data for minors, CCHC is configured to allow parents access to his/her child up to, but not including, age 10. Once the child turns 10, the parent will no longer be able to access the child’s record in CCHC – and will need to contact the healthcare provider directly to request copies of records. Please call or email the CCHC Help Desk and ask for assistance with adding a “dependent account” to your current primary CCHC patient portal account if you have a child under 10.
